Declaration of fitness to attend 1:1 sessions.
My child is fit to attend a 1:1 session. If on arrival of the session my child displays a temperature exceeding 98.6°F/ 37.5°C, my child will be taken home immediately. If my child is unwell during the session and displays any COVID-19 symptoms, we will be notified and will collect them immediately. We will then adhere to NHS guidelines. [Please use link below]
Within the last 14 days, my child, nor anyone in our household, is currently displaying any COVID-19 main symptoms: cough, difficulty in breathing, sore throat or a loss or change to your sense of smell or taste.
Within the last 14 days, my child, nor anyone in our household, has had recent exposure to someone who has developed Covid-19 symptoms.
Within the last 14 days, my child, nor anyone in our household, has returned from international travel or stayed in any regions affected with Covid-19.
I understand that Jo Packard has been vaccinated three times and completes bi-weekly lateral flow tests. She has taken all practical steps to minimise any contagion of Covid-19 and will not be held liable if my child becomes infected with Covid-19. Finally, I declare that the information that I have provided is accurate & complete.
